I’ve noticed these for months now, probably a dozen times a day. One ad will start running, and 3 seconds later, a different ad takes it’s place. Annoying as hell as if the ads themselves aren’t annoying alone. Is this intentional? Some kind of programming glitch?

I saw that a lot when I had cable.  Probably a rookie at the control board or in Charter's case...Pure incompetence and a IDGAF attitude.

It's a case of local ad insertion, where a cable or satellite provider will "insert" local commercials for the market that it serves.  Those ads don't always sync with the ones provided by the network itself.

I experience it frequently on a local station where I watch the news. But it is not only commercials. I sometimes think their studio technicians are the Three Stooges. Mistakes range from microphones turning off, mismatched photos/videos with the story being reported, previous days high afternoon temperatures being displayed as the current 6:00 AM temperatures, and far to often the screen going black with no audio for as long as 5 minutes which my inside joke is someone tripped over the cord and pulled the plug again.

Yes, that's the cable operator not inserting local ads on time. I used to see this all the time. Always fun to see a direct-response ad interrupted...like DealDash and the woman who 'won' that HDTV. 'I WON THIS 55 INCH-' Sometimes I'd see the first few seconds of a Colonial Penn ad, or Future Buildings, or the Addiction Network. Especially love seeing those stupid Addiction ads being interrupted! Smile
Local stations are mainly ran by automation nowadays, and some are ran from hundreds of miles away. Sometimes I see the anchors still finishing up their newscast 'that's all the time we have-' and BOOM a commercial comes on. One time they were still in the middle of WEATHER when the automation kicked in at the end of the newscast.

This used to happen CONSTANTLY when I had Comcast. (evil b*stards)

It does happen with Fios from time to time...usually on sloppy editing on national news channels. Actually sometimes it's a blessing. An overplayed annoying car commercial might start, then immediately gets cut off to a commercial for the local news showing what's coming up at 5pm.

Nobody really CARES anymore. I was in broadcasting 30 years ago, and we took pride in running a "tight" sound, with seamless editing. These days, they just throw the stuff in.

Just saw a Deal Dash ad being cut off by an InventHelp ad which I don't think is local.

The InventHelp ad may have been inserted by the cable company nevertheless. They too receive direct-response commercials. The DealDash ad came off the raw satellite feed of that network. Even DirecTV/Dish Network sometimes insert direct-response ads over the national feed.

Talking of cut-off ads. I had to look up a Jimmy John's ad on YouTube because it constantly got cut at the end! Seriously WTF!?
One good thing about TV-you could always turn it off

Here's a very well-done example of that, IMO-- this 6 P.M. report from WCBD Channel 2 in Charleston, SC (this when that was the ABC station of that area; the news title at this time was TV-2 Eyewitness News); Jill Franco was the main anchor, Lisa Willis had weather and Garry Gupton was on sports.
